Electronic Proxy Forms must be completed NO LATER than 24 Hours before a scheduled meeting so that the proxy can be verified with the member by an officer of the committee.

If you want to give a proxy form to a proxy to bring to an in-person meeting, you can also download the proxy form in PDF format, complete the form, sign and witness and send with your proxy.
